Introduction
This user guide provides an overview of how to use LionSafe General Requests. Examples of General Requests include reporting general safety concerns as well as requesting general services, consultation, training, or other supply items from EHS. It could take EHS 24 to 48 hours to review the request and assign it to the appropriate EHS staff. If you require immediate assistance, please complete the form, and then call EHS to expedite the request.
Do not use the General Request form to report an emergency, incident, injury, or waste request. For more information, please refer to the TYPES OF EHS REQUESTS in the What You Need to Know section of this User Guide.

What you Need to Know
This User Guide applies to the Penn State Community, Faculty, Staff, and Students. The General Request is not an anonymous request. The system will use your Penn State login to identify you as the requestor.
Types of EHS Requests
Accident / Injury Report
Do not report in LionSafe. Follow Penn State POLICY SY04 EMPLOYEE ACCIDENTS - REPORTING AND INVESTIGATION. For more information on the Penn Sate Policies for Accidents and Injuries go to https://ehs.psu.edu/accident-reporting-and-investigation.
Anonymous Report Contact Form
If you would prefer to make an anonymous request, please go to https://ehs.psu.edu/anonymous-report-contact-form and fill out the Anonymous Report Contact Form.
Emergency
Do not Report in LionSafe, call 911 for any life-threatening emergency such as fires, explosion, and serious injuries, and any other incident which poses an immediate threat to life or the environment. For more information, please go to https://ehs.psu.edu/case-emergency
General Requests, for Reporting Safety Concerns and Requesting EHS Services

Incidents: Event Report
An unexpected event that does not lead to injury or illness but results in or could have reasonably resulted in a spill/release, equipment/property damage, environmental impact, or other loss. To Report an Incident, use the LionSafe Event Report form. To learn more about LionSafe Event Report go to https://ehs.psu.edu/lionsafe/training.

Waste Request Forms
To better serve the University community, the waste disposal program is divided by the type of waste to be properly managed. This includes radioactive waste, chemical waste, infectious waste (regulated medical waste), light tubes and ballasts (universal waste), battery recycling, and DEA controlled substances disposal (Contact EHS directly at 814-865-6391).
For waste disposal (except for infectious waste) at campuses other than University Park, please contact your Campus EHS Regional Coordinator.
