Introduction

This description will describe the Company Information section of the NDA request form.

"Company Information" is the second section of the Non-Disclosure Agreement Request Form. 

A Company Name and full address are required for an NDA Request to be submitted.  For foreign based companies (Country is not United States), State and Zip are not required.

A user can begin typing in the "Company Name" text box to trigger the type ahead feature, which populates a drop-down list of suggested values as the user types.  A minimum number of three characters must be typed before the drop-down list will appear.  If the user sees the company name in the drop-drop list for which they are seeking, they can select it to populate the other fields on the form.  An address will be populated if found, as will the company website.  Not all companies have this information stored. 

When validation is successful the search icon will turn to a green check mark.


Users can also enter companies that have not been validated, or do not exist in SIMS.  The icon will remain as a blue search icon to show that the company was not validated.  Users will need to provide the address and website in addition to the contact information.  


Required fields are indicated with asterisks and yellow shading.
